Disability certificate and special CNIC in Sindh
Losing a leg entitles a person in Pakistan to a number of rights and concessions, in employment, education, travel and welfare support. Almost all of them ask for the same two documents first. The process takes patience, and it is worth doing once, properly.
How do you get a disability certificate in Sindh?
In Sindh the certificate is issued through the Department of Empowerment of Persons with Disabilities, which has a head office in Karachi and offices in the districts. You submit a form with identity documents, photographs and medical records, attend a medical assessment board, and receive a disability assessment certificate. With that certificate you then apply at a NADRA centre for a national identity card carrying the disability logo.
Four stages
- Stage 1
Get the form and gather documents
From the department's office or website. Identity card, or B form and a parent's card for a child, photographs, proof of age, education certificates, and medical records of the amputation.
- Stage 2
Submit and receive a board date
A relative should go with the applicant. You are given a date for the medical assessment board, held at a designated government hospital.
- Stage 3
Attend the medical board
Doctors examine the applicant and record the nature and extent of the disability. Bring the discharge slip, any X rays and a letter describing the prosthesis.
- Stage 4
Certificate, then NADRA
Collect the certificate, then visit a NADRA centre in person with it to apply for the identity card with the disability logo.
Offices, forms, fees and waiting times change. Please confirm the current procedure with the department before you travel to an office, and keep photocopies of everything you submit.
Why it is worth the effort
Employment
Reserved quotas in government and many private sector jobs for persons with disabilities. Employers ask for the certificate and the special identity card.
Education
Fee concessions, reserved seats and age relaxations at many public institutions.
Travel
Fare concessions that have been offered on rail and air travel to holders of the special card.
Welfare support
Eligibility for assistive devices, stipends and grants from government welfare bodies and many charitable trusts.
Vehicles
Concession schemes have existed for adapted vehicles. Ask what currently applies.
Legal standing
Formal recognition under provincial disability law, which matters in any dispute over rights at work or in education.
Entitlements are set by the federal and provincial governments and are revised from time to time. Treat this list as a guide to what to ask about, not as a promise.
The papers we can give you
Medical boards, employers, insurers and welfare bodies all want the same thing from the clinical side: a clear, signed description. We provide these without charge to our patients.
- A letter stating the level and side of amputation, the date and the cause as recorded in your hospital papers
- A description of the prosthesis prescribed and what it allows you to do
- An itemised quotation, where a body is considering funding a leg
- A functional note for an employer: what you can do now, what to avoid for the time being, and for how long
Ask at any visit, or message us and collect it.

What families tell us helped
Go early in the day, and go with a relative. Carry originals and several photocopies of every document. Wear the prosthesis, and also be ready for the board to ask to see the stump. Be patient and polite, and keep a record of who you spoke to and when.
Organisations in Karachi that work with persons with disabilities often run help desks and registration drives, and can guide an application that has stalled. If an office refuses to process a complete application, complaints can be lodged with the relevant ombudsperson.
For patients outside Sindh, the certificate is issued through the social welfare department and district medical boards of your own province. The NADRA stage is the same everywhere.
Does wearing an artificial leg affect my eligibility?
No. The certificate records the amputation, which is permanent. Walking well on a prosthesis does not remove the disability in law.
Do I need the certificate to get a leg from you?
No. It has nothing to do with treatment. It matters for jobs, education, concessions and funding applications.
Can a child be registered?
Yes, using the B form and a parent's identity card. It is worth doing early, because school concessions and later quotas depend on it.
We live abroad. Can we apply for my father from overseas?
The applicant must attend the medical board and the NADRA centre in person in Pakistan. A relative in Pakistan can collect the forms and accompany him.
Will a charity fund a leg once he has the certificate?
Some welfare bodies and trusts do contribute, and most ask for the certificate, the special identity card and an itemised quotation. We supply the quotation and a prescription letter for any application you make.
