Prosthetic knee joints, explained

Your natural knee is controlled by muscles that react in a fraction of a second. An artificial knee has no muscles. Its design decides whether it holds when you stumble on a broken slab, and that matters far more than its brand or country.

How does a prosthetic knee work?

A prosthetic knee has two tasks. In stance, when your weight is on the leg, it must stay straight so you do not fall. In swing, when the leg is in the air, it must bend and straighten in time for the next step. Simple knees do this with locks, friction and geometry. Advanced knees use hydraulic cylinders, and the most advanced add sensors and a small computer.

Two ideas

Stance control and swing control

Stance control is about safety

It is whatever stops the knee folding under load: a lock, a brake that grips when weight is applied, a linkage whose geometry keeps it straight, or hydraulic resistance that lets the knee yield slowly instead of collapsing.

Swing control is about walking speed

A knee with simple friction swings at one speed. Walk faster and the shin arrives late, so you wait for it. Pneumatic and hydraulic cylinders adjust resistance as you change pace, so the foot is always there in time.

An older person who walks slowly at home needs excellent stance control and hardly any swing control. A young man who walks to work and climbs stairs needs both. This is the whole logic of choosing a knee.

Honest advice

Before you buy a microprocessor knee in Pakistan

Electronic knees are remarkable. The evidence that they reduce falls is good, and they help unsteady users as much as athletes. If the family can fund one, it is a real benefit. But ask these questions first, of us or of anyone.

  • Who services it, and where? Many units must be sent abroad for service. Ask how long you will be without it, and whether a loan knee is provided.
  • Is the warranty valid in Pakistan? Ask for the serial number and the warranty card in your name.
  • Is it new? Used and refurbished units are sold as new. A genuine unit has traceable paperwork.
  • Can you charge it every night? Think about load shedding and travel to your village.
  • Is the socket right first? Never fit an electronic knee under a socket that is not already comfortable.

Useful extras

Small additions that matter in a Pakistani home

Rotation adapter

Fitted above the knee, it lets the shin turn sideways at the press of a button. It makes sitting cross legged on the floor, changing shoes and getting into a car far easier.

Shock absorbing pylon

Takes the jolt out of each step. Helpful for sensitive stumps and for the lower back.

Knee cover or protector

Keeps dust out of the mechanism and stops the knee wearing through your trousers.

Care

Looking after a knee joint

A mechanical knee is a machine with bearings, bushes and springs. In a dusty, humid city it needs attention once a year, the same way a motorcycle does.

  • Bring it for a service every twelve months, sooner if you walk a great deal.
  • New noises, a wobble side to side, or a knee that swings differently all mean it should be seen now.
  • Keep it dry. Wipe it after rain. Do not oil it yourself, because the wrong oil collects grit.
  • Respect the weight rating, including anything heavy you carry.
  • If it has a lock or a brake, test it against a table each morning before you trust it.

Questions

Knee joint questions

Ask your own question on WhatsApp

Which knee is safest for an elderly person?

A manual lock knee is the safest of all because it cannot bend while walking. A weight activated safety knee is the next step for someone steadier. A microprocessor knee is also very safe if it can be funded and serviced.

Is a locked knee a bad choice?

No. For a frail person it is often the right one. Walking looks stiff, but a walk without falls is better than a natural looking walk that ends in a hip fracture. It can be changed for a free knee later.

Can I climb stairs step over step?

Going up, almost everyone leads with the good leg one step at a time. Coming down step over step is possible with hydraulic and microprocessor knees that yield under load, after training.

How long does a knee joint last?

A good mechanical knee lasts three to five years with yearly servicing, often longer for light users. Hydraulic units may need new seals. Electronic knees follow the maker's service schedule.

Can my knee be upgraded later without a new socket?

Yes. Knees attach by standard adapters. The leg is realigned after the change, and you will need a few training sessions to learn the new knee.
